PDPA ARTISTE CONSENT RELEASE FORM

The Applicant understands that by submitting this form, the Applicant agrees and gives consent to Darren Tan Legacy Planning Lab Pte Ltd for the collection, use and disclosure of the Applicant’s personal data declared in this form for the agreed purpose, record keeping, statistical and research purposes. Darren Tan Legacy Planning Lab Pte Ltd shall ensure that it follows strict security procedures for obtaining, processing and storing the personal information with which it has been entrusted with. Darren Tan Legacy Planning Lab Pte Ltd shall not disclose or make the Applicant’s personal information available to a third party without consent, unless required to do so by law. Darren Tan Legacy Planning Lab Pte Ltd will correct or delete any inaccurate information that it is holding about the Applicant that is brought to its attention.

The Applicant is aware that the event may feature private companies or its representatives presenting private equity opportunities to the Applicant and inviting the Applicant to consider the purchase of such private equity. The Applicant is aware that there are inherent risks in participating in such offers and that such offers are not made by Darren Tan Legacy Planning Lab Pte Ltd and Darren Tan Legacy Planning Lab Pte Ltd is not responsible for the contents or viability of such offers.

ARTISTE’S RELEASE

The undersigned, do hereby expressly and irrevocably grant to Darren Tan Legacy Planning Lab Pte Ltd as the producer of the Event, which includes successors and assigns, and without any further consideration from Darren Tan Legacy Planning Lab Pte Ltd, the permission to:

  • film and/or videotape and/or record and/or photograph me in connection with my participation in the Event, and to publish, exhibit, perform, submit for competition, broadcast, transmit, reproduce, distribute, sub-license and sell the resulting film(s), videotape(s), recording(s) and photograph(s) (collectively, “Work”) and/or Event, in any and all manner and media, whether now know or later developed, throughout the world, in perpetuity;
  • prepare, publish, exhibit, perform, submit for competition, broadcast, transmit, reproduce, distribute, sub-license and sell derivative works based on the Work and/or Event; and
  • use my name, likeness, voice, photograph and information about me in the Work and/or Event, and derivative works based on the Work and/or Event, in any and all manner and media, whether now known or later developed, as desired by the Darren Tan Legacy Planning Lab Pte Ltd, including but not limited to promoting and advertising the same.

I understand that in proceeding with Work, the Darren Tan Legacy Planning Lab Pte Ltd is doing so in full reliance on the foregoing permission.
